I think it is wise to take the precautions you mention here but I really think that the tag lists of fellow authors at the bottom of their stories is what is causing the problem - if you read Harry's post of yesterday where he was reinstated -

You will see it clearly when Medium state 'Continuing to bulk at-mention (which is tagging) other authors and solicit reads and interactions is what triggered our filters again' Bulk tagging is dangerous guys! Don't tempt fate and those Medium filters......
